Welcome to the Medina Central School District! To begin the enrollment process, please access the required forms below or pick up a packet at the Business Office located in the District Office Building @ 1 Mustang Dr.
When the packet is completed and all documents are gathered, please contact the building principal to schedule an appointment.
Please be sure to bring with you the following documents:
- Proof of each child’s age (original birth certificate or passport)
- Immunization and health records
- Proof of residency (lease, mortgage statement, current utility bill or tax bill)
