If your child has a Seizure Disorder, a Seizure Disorder Emergency Care Plan is required to be completed by your child's provider and returned to the school nurse.  It is to be updated at the start of every school year.  If you are unable to print this form from home, please contact your school nurse and she will be able to get one for you.
